The following template should be used when submitting an attendance verification request:
- Student Name
- NSU ID
- Term
- CRN
- Class Prefix and Number
- Section Number
- Professor's Name

Class attendance request for all courses taken at the main campus excluding courses Master's of Accounting and Taxation programs will be sent to Melissa Madani Academic Services Coordinator.

Class attendance request for classes in the Master's of Accounting and Taxation programs will be directed to the Assistant Director for MACC/MTAX.

Class attendance request for online courses will be sent to Kelly Duke Assistant Director.
