All REPs and supporting documentation needs to be submitted to electronically for review. The subject line of the email should indicate as follows:REP and the student’s name. The email should contain a the electronic copy of the REP and Any supplemental documentation (medical notes, death certificates, etc.) pertaining to that student’s issue needs to be scanned in and attached to the same email. Please remember to let your students know that REPs may take up to 4 weeks for a decision to be made. Also, when you submit an REP, please make a notation in SPACMNT indicating that there is an REP in the works. The “RATIONALE” section of the REP should be populated with whatever the student has submitted to the advisor in writing (e-mail, letter, etc. is copied into this space). Academic advisors will provided feedback in the in the “Advisor Recommendation” section under “FOR OFFICIAL USE ONLY.” ARCHIVE

Denia Lee-Hing will be the initial contact person for any requests. Please submit all REP documentation and paperwork to Denia via email. The REP document will need to still be in MS Word (so we can edit, modify, etc.) and any documentation should be scanned and sent attached to the same email. All documents for an individual student should be kept together. Denia will do an initial check for accuracy and review the documentation submitted. If Denia notices that a “standard” document request has not been included (e.g. an attendance report), she will request that you obtain it at that time. Denia will meet regularly with the Academic Advising Manager to review REPs and supporting documentation. If a decision can be made at that point, Denia will follow up with you regarding the decision. If further questions or issues arise at the point of review, Denia will let you know what further documentation may be requested. If there are other steps required (e.g. Involve Steve, program office, or another member of the management team) prior to making a decision, Denia will update you at that time.
