Utilizing Program Evaluation Forms

Please use these forms each time you do a presentation or program (ex. PES, Orientation, Lunch ‘n Learn, etc.).

There is a paper version for handing out, and an electronic version for emailing to students. Please make sure to maximize return rate by encouraging students to complete them. If possible, you may wish to fill out the presenter’s name and program name in advance, if you have time. Don’t worry if you don’t.
