Exit Survey: BAC Attrition

The OAA Exit Survey will be completed for the following two reasons:

• Student is withdrawing from Huizenga entirely
• Student status: Inactive for 3 consecutive terms. (Student must have been active sometime within the allotted 4 terms)

When documenting student records the appropriate BAC Code will need to be entered into SPACMNT. For a listing of BAC Codes please see the Advisor Comment Key 3.13.16

Please complete the Exit Survey when speaking with students who meet the criteria listed above. Data entered into the survey will be gathered in a data base for review.
